Musk reportedly asked employees not to wear safety vests and failed to separate dangerous areas from workspaces. The investigation also found that over 600 workers have been injured since 2014. Safety measures, such as tie-downs for transporting cargo, were lacking, leading to incidents like the death of an employee.
