A Nottingham coroner has said "missed opportunities" by the probation service had "significantly contributed" to the murder of Janet Scott.
The mum-of-six was murdered in 2018 by Simon Mellors - who had already served a life sentence for killing his previous partner.
Although he was out on license, his probation officer failed to tell police that he had been stalking Janet Scott after she ended the relationship.
The inquest into Mrs Scott's death found the probation officer managing Mellors failed to spot vital signs that he was a danger, and had he acted sooner Mrs Scott might be alive today.

The parents of Grace O’Malley-Kumar have blamed “failings” in the system for a series of stabbing attacks, including a sword attack in Hainault on Tuesday and a school stabbing in Sheffield on Wednesday. Grace’s father, Dr Sanjoy Kumar, said “positive change is so long overdue” after a 14-year-old boy was killed in Hainault and three teenage boys were admitted to hospital in Sheffield following the attacks.

An ex-prisoner has been sentenced to an indefinite hospital order for killing an elderly mobility scooter rider. Lee Byer, 45, was psychotic when he stabbed 87-year-old Thomas O’Halloran in Greenford, west London, in August 2022. Mr O’Halloran, originally from County Clare, was well-known in the Greenford community. Byer, who had had a string of convictions, was released from Wormwood Scrubs five days before the killing. Mental health reports found Byer was psychotic, hearing voices, and suffering from paranoid schizophrenia. Report by Jonesia.
