Mafia: The Old Country is set in Sicily, Italy, during the 1900s in the fictional town of San Celeste, which was previously featured in Mafia 2.
It is based around the origins of the Sicilian Mafia.
The narrative follows Enzo Favara who journeys from a childhood of indentured labor as a carusu in Sicily's sulfur mines to joining Don Torrisi (Johnny Santiago), the head of the Torrisi crime family.
